- Workplace and school accommodation documentation, usually following an adult ADHD or autism assessment with me
For clients I see in ongoing therapy or have assessed. Documentation is written from an actual assessment and treatment relationship, which is what employers, landlords and licensing boards expect.
For FMLA and ESA documentation, you need to be in therapy with me at least once a month. I can only document a condition and a need that I have assessed and continue to see. That protects you if the paperwork is questioned, and it protects my license.
If you are new, we start with a free consult and an initial assessment. Documentation comes once I have enough clinical information to write it honestly.
I do not provide same-day letters, letters without an assessment, or letters for people I am not treating. I cannot guarantee that an employer, school or landlord will approve a request. If I do not think documentation is clinically appropriate, I will tell you why.
Paperwork and letters are separate from sessions and cost $150 per purpose. For example, an FMLA form and an ESA letter are two purposes. Therapy sessions are billed at the usual rate. See all fees.
No. I only write ESA letters for clients I see in therapy at least once a month, so the letter reflects real, ongoing clinical care.
